One of the following options, as determined by your collective agreement:

• 20% paid by members and 80% paid by the plan
• 10% paid by members and 90% paid by the plan
• 5% paid by members and 95% paid by the plan

You will receive a 5% reduction on the member-paid portion if you purchase your prescription drugs at one of the preferred provider pharmacies listed below.

If you wish to order your prescription by mail order, you may contact: MediTrust Pharmacy at 1–800–263–8999 or Pharmex Direct at 1–800–663–8637.

If you require maintenance drugs, we urge you to purchase a three-month supply of medication at one time. This will also help to contain costs (and save you money) as only one dispensing fee is charged, rather than three or more.
